About the Role:

LHC Group is seeking a skilled Registered Nurse (RN) to fill the position of Care Coordinator. As a key member of our team, you will be responsible for conducting assessments, developing care plans, and providing ongoing monitoring of long-term care insurance claimants.

Key Responsibilities:

  • Conduct telephonic/virtual assessments to collect information about clients' functional, cognitive, and medical status.
  • Review assessments completed by field assessors for quality, obtaining clarification as needed.
  • Evaluate the plan of care to determine if the appropriate care is in place to ensure safety and quality of life for clients.
  • Develop appropriate care plans utilizing knowledge of community resources and services, recommending services that best meet clients' needs.
  • Maintain quality standards as established by LTS.
  • Recognize and communicate problems/issues immediately for rapid resolution.
  • Participate in Quality Improvement activities.
  • Attend staff meetings, educational programs as directed.

Requirements:

  • Registered Nurse graduate of an accredited college or university.
  • Experience in conducting social and health assessments.
  • Understanding of geriatric care.
  • Computer literacy required.
