Healthcare Support is seeking a highly skilled Physical Therapist to join our team at a Skilled Nursing Facility in Ripley, Ohio.
Job Summary:We are looking for a dedicated and compassionate Physical Therapist to provide high-quality patient care in a fast-paced Skilled Nursing Facility setting. As a Physical Therapist, you will be responsible for evaluating patients, developing treatment plans, and implementing interventions to improve mobility and function.
Responsibilities:- Evaluate patients to identify physical therapy needs and develop individualized treatment plans
- Implement interventions to improve mobility, strength, and function, including exercises, modalities, and education
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care
- Document patient progress and outcomes in a timely and accurate manner
- Participate in interdisciplinary team meetings to discuss patient care and develop treatment plans
- Current Physical Therapy license in the state of Ohio
- 1+ year of experience as a Physical Therapist in a Skilled Nursing Facility setting
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Commitment to providing high-quality patient care
