Job Summary:

Responsibilities:
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient goals and objectives.
  • Provide hands-on therapy sessions to patients, focusing on improving mobility, strength, and function.
  • Collaborate with interdisciplinary teams to ensure seamless patient care and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • BLS certification from AHA (American Heart Association) required.
  • 1+ year of recent experience in a Skilled Nursing Facility setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
