Job Summary:The Nurse Liaison is responsible for providing on-site consultative services to area hospitals and facilities, developing comprehensive adult and geriatric assessments, and coordinating homecare or post-acute skilled nursing needs. This role requires strong clinical assessment skills, excellent communication, and the ability to work independently.
Responsibilities:- Provides on-site consultative services to area hospitals and facilities regarding adult and geriatric patients referred for homecare services.
- Develops comprehensive adult and geriatric assessments to assure development of thorough homecare plans or discharge plans to Concordia Visiting Nurses.
- Coordinates homecare or post-acute skilled nursing needs to assure quality patient care and delivery of services.
- Attends interdisciplinary clinical meetings at institutions as requested.
- Documents patient information and daily activities to Concordia Visiting Nurses branch staff via the contact relationship management software (CRM).
- Identifies, develops, and builds upon referral sources within assigned institutions.
- Educates markets and promotes to patients, families, and referral sources the Concordia Lutheran Ministries continuum of care.
- Current Pennsylvania Nursing License: Registered Nurse or Licensed Practical Nurse.
- Bachelor's degree in nursing recommended.
- A minimum of three (3) years nursing experience required. Supervisory and acute care discharge planning experience preferred.
- Case manager certification and/or specialty certification preferred.
- Maintains current PA Driver's License and auto insurance.
