Job Title: Infusion Nurse

Concordia Visiting Nurses is a Medicare-certified, state-licensed, and Medicaid-approved home health agency operating in nine Western Pennsylvania counties. Our goal is to deliver high-quality home health services to our patients with respect, dignity, and compassion.

Job Summary:

We are seeking a skilled and compassionate Infusion Nurse to join our team. As an Infusion Nurse, you will provide high-quality patient care, administer intravenous therapy, and work collaboratively with our interdisciplinary team to achieve optimal patient outcomes.

Responsibilities:
  • Administer intravenous therapy to patients in their homes
  • Conduct thorough assessments and develop individualized care plans
  • Collaborate with the healthcare team to ensure seamless care transitions
  • Provide education and support to patients and their families
  • Participate in quality improvement initiatives to enhance patient care
Requirements:
  • Graduate of a state-approved nursing program
  • Current licensure in the State of Pennsylvania
  • Minimum of two years of experience in patient care
  • Reliable transportation and a valid driver's license
  • Previous experience with IV therapy
