Tact Staff is currently seeking a travel nurse RN specializing in Hospice for an exciting travel nursing opportunity.

Key Responsibilities:

  1. Conduct comprehensive assessments of patient and caregiver needs to deliver high-quality care aligned with evidence-based practices and regulatory standards.
  2. Develop and communicate an actionable care plan for each patient, ensuring inclusion of patients and caregivers in the planning process.
  3. Act as Case Manager for assigned patients, managing caseloads and visit schedules according to agency productivity benchmarks.
  4. Complete OASIS documentation accurately and timely, adhering to agency protocols and regulatory requirements.
  5. Implement medical plans of care and nursing interventions to ensure safe and effective patient care.
  6. Maintain proficiency in advanced nursing skills, including IV therapy and complex wound care, while adhering to infection control standards.
  7. Collaborate with LPNs to ensure adherence to care plans, delegating tasks as appropriate.
  8. Evaluate the effectiveness of care plans, making adjustments as necessary to meet patient needs.
  9. Ensure accurate and timely documentation of nursing care to facilitate effective communication within the healthcare team.
  10. Provide education to patients and caregivers regarding treatment options, medication management, and discharge instructions.
  11. Identify interdisciplinary needs and communicate them effectively to relevant team members.
  12. Complete necessary agency and regulatory forms in a timely manner during patient visits.
  13. Conduct visits efficiently, ensuring supervisory visits are appropriately scheduled.
  14. Assess patient needs for home care assistance and coordinate plans of care accordingly.
  15. Report patient incidents promptly to the Quality Improvement Specialist.
  16. Facilitate safe patient discharges and transfers, ensuring all documentation is processed correctly.
  17. Coordinate care with physicians and other healthcare providers to support the recommended care plan.
  18. Participate in peer review audits and provide constructive feedback.
  19. Schedule and facilitate case conferences to enhance team communication and documentation.
  20. Advocate for patient care issues to ensure quality and individualized care.
