Job SummaryWe are seeking a highly skilled and compassionate Primary Care Physician to join our team. As a Primary Care Physician at ChenMed, you will be responsible for providing high-quality, patient-centered care to our senior patients. You will work closely with our care teams to develop and implement personalized care plans that address the unique needs of each patient.
Key Responsibilities- Provide comprehensive primary care to patients with acute and chronic illnesses, including geriatric patients.
- Develop and implement evidence-based treatment plans that address the unique needs of each patient.
- Work closely with our care teams to coordinate patient care and ensure seamless transitions between care settings.
- Participate in quality improvement initiatives to improve patient outcomes and reduce healthcare costs.
- Stay up-to-date with the latest medical research and best practices in primary care.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred.
- Current, active State Medical License for the state(s) in which you will be working.
- Ability to obtain and maintain malpractice insurance.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
