Overview

St. Luke's Health system is seeking a highly skilled Nurse Practitioner or Physician Assistant to spearhead the development of our innovative Hospital at Home program. This forward-thinking care model enables hospital-level services to be delivered directly to patients' homes, resulting in significant cost savings, improved patient outcomes, and enhanced patient satisfaction.

The Advanced Practice Provider will collaborate closely with operational partners, working under the guidance of physicians to focus on clinical outcomes, program evolution, operational efficiency, and inter-provider collaboration. This role will involve supporting inpatient, virtual, and home-based patient visits, while contributing to the growth and success of the program.

Qualifications
  • Graduate education from an accredited Nurse Practitioner or Physician Assistant program, as designated by the Idaho Board of Nursing or Medicine
  • Prior experience in Home Health or Hospitalist Advanced Practice Provider roles is strongly preferred
  • Licenses and certifications required for the specialty, with eligibility to obtain licensure from the Idaho Board of Nursing or Medicine
