About the RoleThe **Nurse Advocate** will play a critical role in developing and executing strategies to improve health outcomes for CINQCARE's members. This includes working closely with our interdisciplinary team to design, develop, and execute care pathways, as well as deploying remote patient monitoring and patient self-reporting tools.
The ideal candidate will have a strong background in nursing, with at least 5 years of relevant clinical experience. They will also possess excellent communication and interpersonal skills, with the ability to build and manage relationships with patients, families, and community partners.
Key Responsibilities- Develop and implement care pathways to improve health outcomes for CINQCARE's members
- Deploy remote patient monitoring and patient self-reporting tools to support care coordination
- Conduct in-home and tele-health assessments to identify patient needs and develop personalized care plans
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
- Provide education and support to patients and families to promote healthy behaviors and self-management
- Participate in quality improvement initiatives to identify areas for improvement and develop strategies for implementation
- Active NY state Registered Nursing license
- At least 5 years of relevant clinical experience
- Excellent communication and interpersonal skills
- Ability to build and manage relationships with patients, families, and community partners
- Strong analytical and problem-solving skills
- Ability to work independently and as part of a team
