At The Jewish Board, we're dedicated to providing compassionate and high-quality mental health services to individuals and families in need. As a Case Manager, you'll play a vital role in helping us achieve our mission.

Key Responsibilities:
  • Conduct comprehensive assessments of clients within 45-60 days of entry into the program
  • Work closely with the team to visit each client a minimum of 6 times monthly
  • Integrate medical, specialized, and behavioral health services with social support and educational services
  • Periodically reassess clients' medical and behavioral health needs and update their care plans
  • Attend scheduled team meetings and one-on-one supervisions
  • Participate in mandatory training sessions and lead team in-service training as needed
Requirements:
  • Bachelor's degree and at least six months of direct care experience with individuals with serious mental illness
  • Ability to work in a team environment and provide care management services
  • Strong communication and interpersonal skills
