General Information:

Job Title: Activity Coordinator

Location: Crest View Nursing & Rehabilitation Center

Schedule: Part-time, 46.15 hours per pay period; shifts from 12:30 PM to 8:30 PM with occasional shifts from 9:00 AM to 5:00 PM. Weekend and holiday rotations are required.

Position Summary:

The Activity Coordinator reports directly to the Recreation Service Coordinator and ensures that residents participate in activities tailored to their physical, mental, social, and spiritual needs. This vital role demands empathy, meticulous attention to detail, and a genuine interest in the field.

Key Responsibilities:

  • Engage with residents daily to enhance their quality of life.
  • Facilitate participation in activities that align with residents' needs.
  • Provide individualized care to residents.
  • Organize and execute activities according to the daily and monthly activity schedule.
  • Lead activities and adhere to care plans to ensure residents attend meaningful activities; provide guidance for various activities.
  • Deliver one-on-one sensory engagement for residents with dementia.
  • Document and record residents' participation in activities.
  • Coordinate food orders to complement scheduled activities.
  • Manage and redirect residents facing behavioral challenges.
  • Maintain cleanliness and organization of workspaces and equipment.
  • Prepare for upcoming events, visits, and group meetings.
  • Perform additional duties as assigned.

Position Requirements:

  • High school diploma or equivalent is preferred.
  • Current CNA certification is preferred.
  • At least one year of relevant experience is preferred.
  • Experience in a healthcare setting is preferred; familiarity with dementia care is a plus.
  • Strong communication skills are essential.

Knowledge, Skills, & Abilities:

  • Intermediate computer skills are required.
  • Self-motivated with excellent interpersonal and problem-solving abilities.
