Job Summary

Honor Community Health is seeking a highly skilled and compassionate Nurse Practitioner to join our team at our School-Based Health Center. As a key member of our integrated healthcare team, you will be responsible for providing high-quality primary medical care to our patients, working collaboratively with our team to create a culture of excellence and dedication to delivering compassionate and high-quality healthcare.

Responsibilities
  • Manage health problems and coordinate primary medical care in partnership with our integrated practice team.
  • Work collaboratively to create and maintain a culture of excellence and dedication to providing compassionate and high-quality healthcare.
  • Responsible for clinical quality and outcomes, as well as productivity.
Requirements
  • Graduate of an accredited Nurse Practitioner program.
  • Current Nurse Practitioner certification in the Family or Pediatric specialty area.
  • Current and valid license to practice medicine issued by the State of Michigan.
  • Current BLS and ACLS certifications through the American Heart Association.
  • Board certified at hire and will be expected to maintain appropriate board certification, licensure, and membership in professional organizations.
  • Eligible to obtain and maintain credentials as a Medicaid and Medicare provider.
  • 1+ years of progressive experience as a Nurse Practitioner.
  • Experience with CPT and ICD coding.
  • Knowledge of and experience with electronic health records (EHR) systems.
