Advanced Practice Provider

Infinite Medical P.C, a nationwide network of advanced practice providers and specialty clinicians, is seeking a compassionate and dedicated Primary Care Nurse Practitioner to join our team.
As a Primary Care Provider, you will play a crucial role in providing comprehensive care to patients in long-term care facilities. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.
Responsibilities:
- Provide comprehensive healthcare services and coordinate patient care
- Work closely with a team of healthcare professionals
- Assess, diagnose, and treat residents' physical and mental well-being
- Conduct thorough patient assessments, including medical history reviews and physical examinations
- Order and interpret diagnostic tests
- Develop accurate diagnoses and treatment plans
- Prescribe medications and administer treatments
- Provide counseling to residents and their families
- Collaborate with the interdisciplinary team for seamless coordination of care
- Manage chronic conditions and monitor progress
- Make appropriate adjustments to care plans
- Commitment to person-centered care
- Create a safe and nurturing environment for elderly residents in the nursing home.
Requirements:
- Nurse Practitioner or Physician Assistant License
- DEA License
- 6 months experience in long-term care

Salary Range: $150,000.00 - $200,000.00
