Candidate Profile

The ideal candidate for the Practice Transformation Facilitator role will possess the following qualifications:

  • Education: A Bachelor's degree or equivalent with a strong academic track record.
  • Experience: A minimum of 3 years in clinical practice, with 5+ years of relevant experience in the healthcare sector preferred. Previous roles in primary care settings, particularly with successful quality improvement initiatives, are advantageous.
  • Entrepreneurial Spirit: A commitment to addressing longstanding gaps in care delivery for Black and Brown populations. The candidate should demonstrate urgency and results-driven execution in a dynamic healthcare landscape.
  • Communication Skills: Exceptional verbal and written communication abilities, with a talent for presenting complex concepts clearly to diverse audiences, including team members, investors, and community partners. Proficiency in Microsoft Office is essential.
  • Relationship Management: Proven ability to cultivate and maintain effective relationships with business leaders and external stakeholders.
  • Organizational Culture: Strong ethical judgment, teamwork orientation, and a desire to thrive in a fast-paced, entrepreneurial environment, with a commitment to organizational values such as integrity, scholarship, and interdisciplinary collaboration.

Role Responsibilities

The Practice Transformation Facilitator will report to the Practice Transformation Manager and will be responsible for providing strategic insights, organizational skills, and evidence-based analysis to guide decision-making and fulfill CINQCARE's objectives. The role embodies CINQCARE's core values: Trust, Empathy, Commitment, Humility, Creativity, and Community Focus.

The key responsibilities include:

  • Collaborating with a team of Practice Transformation Facilitators across various healthcare settings.
  • Assisting practices in maximizing available technology resources to improve patient outcomes and operational efficiencies.
  • Working with interdisciplinary teams to identify opportunities for optimizing technology resources, including:
    • Strategies for population health management
    • Registry and enrollment functionalities
    • Care coordination workflows to enhance patient care
    • Implementation of care management strategies
    • Development of patient action plans
    • Collection and reporting of performance data
    • Implementation of patient safety initiatives
    • Facilitation of community learning forums
  • Acting as the primary liaison between healthcare organizations and external stakeholders.
  • Providing guidance to care teams in adopting best practices and implementing quality improvement strategies.
  • Building trust-based relationships with healthcare practices to support change management and sustainability of initiatives.
  • Delivering performance reports to healthcare practices, identifying gaps, and proposing solutions.
  • Prioritizing quality and utilization initiatives critical to value-based care success.
  • Leading practice transformation forums to foster collaboration.
  • Offering training on technology tools to healthcare teams.
  • Ensuring compliance with regulatory standards.
  • Documenting and assessing practice progression throughout the transformation process.

Leadership and Strategy

The Practice Transformation Facilitator will lead in defining and executing strategies to create business value in clinical settings, collaborating with teams to design and implement these strategies effectively.

Collaboration and Knowledge Sharing

This role will ensure that clinical capabilities are cohesive and will work closely with other divisions to understand their needs and define solutions that align with CINQCARE's business objectives.

Work Environment

This position requires on-site work in a traditional office setting. Effective communication with various stakeholders is essential, and the role may involve extended periods of sitting and the occasional lifting of light objects.
