Primary Care Nurse III

Columbia University Irving Medical Center is seeking a highly skilled Primary Care Nurse III to join our team. As a key member of the patient care team, you will provide exceptional care to pediatric and adult populations, working collaboratively with healthcare professionals to deliver high-quality patient outcomes.
Responsibilities:- Execute nursing interventions by collaborating with the care team and following established standards of care.
- Assess and systematically collect data related to patient health status, working with the care team to develop comprehensive plans of care.
- Utilize data to continuously meet care coordination needs across settings, in conjunction with patients, families, and other care providers.
- Requires a bachelor's degree in Nursing or equivalent education, training, and experience, plus four years of related experience.
- Current and unrestricted New York State RN license.
- BLS certified.
- Ability to work collaboratively with a culturally diverse staff and patient/family population, demonstrating tact, respect, and sensitivity.
- Excellent communication skills and the ability to communicate effectively with a diverse audience of peers, management, and other stakeholders.
- Physician practice experience preferred.
