Primary Care Nurse II

Columbia University in the City of New York is seeking a highly skilled and dedicated Primary Care Nurse II to join our team at Columbia Primary Care at Columbia University Irving Medical Center (CIUMC).
Job Summary:The Primary Care Nurse II will report directly to the Director of Nursing, Columbia Primary Care, and will be responsible for providing high-quality patient care to pediatric and adult populations. This role will involve performing nursing duties consistent with the NYS nursing scope of practice, including medication administration, point of care testing, clinical information abstraction, and patient education.
Responsibilities:- Execute nursing interventions by collaborating with the care team and following the standards of care established by Columbia Primary Care.
- Assess and systematically collect data related to patient health status and work with the care team to develop the plan of care.
- Utilize data to continuously meet care coordination needs across settings in conjunction with the patient, family, and/or other care providers.
- Assume responsibility and accountability for effectively managing the nursing care of individual patients.
- Document nursing assessment and interventions to reflect a comprehensive and integrated approach to patient care.
- Identify learning needs of the patient and families, educate by adapting to patient and family learning style utilizing the teach-back method.
- Provide care that is safe, ethical, patient- and family-centered, culturally congruent, and evidence-based.
- Bachelor of Science degree in Nursing or the equivalent of education, training, and experience.
- A minimum of 3 years of related experience.
- Current and unrestricted New York State RN license.
- Basic Life Support certified.
- Ability to work collaboratively with a culturally diverse staff and patient/family population demonstrating tact, respect, and sensitivity.
- Must have excellent communication skills and the ability to communicate effectively with a diverse audience of peers, management, and other stakeholders.
- Physician practice experience preferred.
