Behavioral Health Care Manager, Adult

Mental Health Cooperative, a leading provider of mental health services, is seeking a skilled Behavioral Health Care Manager to join our team. As a key member of our care management team, you will play a vital role in providing comprehensive care management services to consumers with severe and persistent mental illness (SPMI) and their supports.
Responsibilities:- Conduct thorough assessments of consumers' strengths, needs, supports, and goals, and develop individualized care plans to address their unique needs.
- Provide support to consumers in their homes, schools, courts, and other community-based locations, as needed.
- Collaborate with consumers, their supports, and other healthcare professionals to ensure seamless care coordination and effective communication.
- Develop and maintain positive relationships with consumers, their families, and other stakeholders to promote successful outcomes and improve quality of life.
- Ensure timely and accurate documentation of consumer interactions, care plans, and other relevant information.
- Bachelor's degree in a health-related field, such as counseling, psychology, or social work.
- Valid Tennessee driver's license and acceptable motor vehicle report.
- Acceptable criminal background investigation and personal automobile insurance.
- Cell phone and transportation that seats four people.
