Medication History Specialist

The Medication History Associate plays a crucial role in ensuring the accuracy and completeness of patient medication records.
Key Responsibilities- Collect and verify patient medication histories for review and reconciliation by the Admitting Physician
- Utilize AllScripts to review patients' home medication and allergy information
- Accurately input patient medications and allergies into the EMR using McKesson
- Collaborate with healthcare teams to ensure accurate medication records
- Research patients' current medications through collaboration with retail pharmacies, nursing homes, families, hospitals, and doctor's offices
- High School Diploma or equivalent required
- Preferred experience: Pharmacy Technician with at least one year of experience in a hospital or retail pharmacy setting
- Knowledge of generic and trade names of medications
- Current American Heart Association Basic Life Support (AHA BLS) course completion card
