Job Description:

As a Registered Nurse (RN) at Commonwealth Home Health, you will play a vital role in delivering high-quality patient care in the comfort of our patients' homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless coordination of care
  • Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and promote patient engagement

Required Skills and Qualifications:

  • Current RN licensure in the state of Kentucky
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to reliable transportation
