Job Description:
The Home Health Nurse Specialist provides quality patient care and technical skilled services in the comfort of the patient's home. This role entails executing services within the scope of practice as defined by state laws governing nursing, adhering to the plan of care, and collaborating with healthcare professionals. Key responsibilities include:
- Documenting progress notes for each visit and submitting them daily
- Observing and reporting patient needs, educating family members on patient care, and maintaining a healthy environment
- Reporting patient responses to treatment and changes in condition to supervising nurses and physicians
Required Skills and Qualifications:
To succeed in this role, you must possess current LPN licensure in the state of practice, a valid driver's license, vehicle insurance, and access to reliable transportation or public transit. CPR certification is also required.
