Job Summary

We are seeking a skilled Social Worker to join our team at Terry Reilly Health Services. As a Social Worker, you will provide on-site behavioral health screening and assessment, crisis intervention, and individual, couple, family, and group counseling. You will also complete treatment and discharge plans in cooperation with medical clinicians and nursing staff.

Responsibilities
  1. Provide on-site behavioral health screening and assessment, crisis intervention, and individual, couple, family, and group counseling.
  2. Complete treatment and discharge plans in cooperation with medical clinicians and nursing staff.
  3. Assess patients' continuing care needs via chart review, direct client interview, and completion of the Treatment Needs Assessment.
  4. Collaborate with other clinic staff in the development and implementation of appropriate treatment plans and interventions.
  5. Generate appropriate medical chart documentation.
  6. Assist patients in the completion of the discharge planning worksheet and provide collaborative linkages with other community programs and services.
  7. Schedule and coordinate aftercare appointments and activities.
  8. Communicate via written report daily discharge planning activities.
  9. Participate in case staffing and consultation with appropriate medical clinicians and nursing staff.
  10. Maintain collaborative linkages with other community programs and services.
  11. Permanently maintain client charts and record progress pertaining to service delivery in a legible, coherent fashion, consistent with established policies.
  12. Supervise patients' scheduled recreation and leisure time activities.
  13. Comply with all current TR policies, including HIPAA policies.
  14. Provide case management services, including completing documentation that would assist clients to access community resources and referring to community services and resources not offered at Allumbaugh House.
  15. Attend and participate in all staff meetings.
  16. Portray professionalism to clients, other TR department personnel, referral sources, and the community during daily interactions and appearances.
  17. Utilize training funds appropriated for educational advancement consistent with CEU policy and attend required workshops as they occur.
Requirements
  1. Successful clearance of Department of Health and Welfare criminal background check.
  2. Masters degree in Social Work or Counseling.
  3. Current professional license in the state of Idaho.
  4. One year work experience in the field of behavioral health.
  5. Understanding and adherence to ethical standards of the respective licensure board.
  6. Good written and verbal communication skills.
  7. Experience working with disenfranchised populations.
Preferred Qualifications
  1. Bilingual in Spanish and English.
  2. Two or more years' experience in the field of alcoholism, substance use disorders, and mental health.
  3. Experience working in a medical setting, crisis mental health, and/or detoxification setting, or as a member of a multidisciplinary team.
  4. Knowledge of computer and email systems, as well as EMR.
  5. Knowledge of community resources.
  6. Familiarity with State of Idaho Health & Welfare Mental Health system.
