Aveanna Healthcare Private Duty Nurse - LVN/LPN

About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team in providing private duty nursing services to our patients. As a Private Duty Nurse, you will be responsible for delivering individualized care to our patients, working closely with our patients and their families to understand their unique needs and preferences.

Responsibilities
  • Implement and manage individualized care plans for patients, working closely with our multidisciplinary team to ensure seamless care coordination.
  • Provide high-quality nursing care to patients, including administering medications, wound care, and other treatments as prescribed by our patients' physicians.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address these needs.
  • Communicate effectively with patients, families, and healthcare team members to ensure that all parties are informed and engaged in the care process.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner, adhering to our company's documentation standards.
Requirements
  • Current, unrestricted LPN/LVN license in the state of practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • One year of experience as a Licensed Nurse in a clinical care setting, with a focus on home health or private duty nursing.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
