Home Health Care Specialist

We are seeking a skilled Senior Community Care Provider to join our team at UnitedHealth Group. This role involves providing care to patients in their homes, nursing homes, and assisted living facilities.

The ideal candidate will have experience in long-term care, geriatrics, and chronic illness management. They will be proficient in documenting medical information with electronic medical records and able to develop and maintain positive customer relationships.

This role offers a competitive salary of $42.31 to $83.27 per hour (Washington Residents Only) and a comprehensive benefits package, including incentives and recognition programs.

About the Job:

  • Coordinate care as members transition through different levels of care and care settings
  • Monitor the needs of members and families while facilitating adjustments to the plan of care
  • Evaluate the effectiveness, necessity, and efficiency of the plan
  • Utilize practice guidelines and protocols established by CCM

Requirements:

Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)Active and unrestricted license in the state which you reside
