Position Overview:
The LPN at Oak Hill's Hebron Location is expected to work 16 hours per week, specifically on Sundays and Mondays from 7 AM to 3 PM. This role involves assisting in the health maintenance and well-being of program participants by monitoring health concerns and implementing policies, procedures, and programs aimed at promoting health and enhancing independence and quality of life, in alignment with Oak Hill's mission.
Essential Qualifications:
- Licensed Practical Nurse Certification
- Valid driver's license

Key Responsibilities:
- Provide assistance, guidance, and instruction in daily living activities, including personal hygiene, meal preparation, and social and recreational activities.
- Attend to the health needs of residents/program participants by maintaining up-to-date knowledge of their health issues.
- Ensure resident/program participant health by contacting physicians in emergencies, documenting incidents, and keeping management informed.
- Facilitate care by recording doctors' orders and maintaining accurate medication records.
- Ensure continuity of care by effectively communicating all medical concerns to incoming shifts, both verbally and in writing.
