- Monitor patients' vital signs and report any changes to RNs or physicians
- Provide wound care and dressings as needed
- Conduct Comprehensive Patient Assessments
- Perform physical exams and medical histories
- Review medication lists and identify potential interactions
- Develop personalized care plans to meet patients' unique needs
- Collaborate with Healthcare Team
- Work closely with RNs, physicians, therapists, and support staff to ensure seamless patient care
- Communicate effectively with patients and families to address their concerns and needs
- Maintain Accurate Patient Records
- Document patients' medical histories, medications, and treatment plans
- Record changes in patients' conditions and administered treatments
- Provide Education and Support
- Educate patients and families on their medical conditions and treatment plans
- Assist patients with self-care practices and promote overall well-being
- Other Duties
- Perform other duties as assigned by management
- Valid and active Licensed Practical Nurse (LPN) state license
- BLS certification
