Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Pine Beach, New Jersey.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work independently to assess and develop individualized care plans, provide manual therapeutic exercises, and educate clients and their families on rehabilitative care and activities necessary for independent living.

Responsibilities:
  • Conduct home visits to assess clients' physical abilities and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Implement manual therapeutic exercises, gait training, and other interventions to promote clients' physical function and independence.
  • Educate clients and their families on rehabilitative care and activities necessary for independent living.
  • Document observations, interventions, and evaluations in a state-of-the-art touch pad tablet.
Qualifications:
  • A current New Jersey Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
