Job Summary:We are looking for a compassionate and dedicated Occupational Therapist to provide home health services to patients in Northern Colorado. As a member of our team, you will work closely with patients, families, and healthcare professionals to develop and implement individualized therapy plans that address the unique needs and goals of each patient.
Responsibilities:- Provide occupational therapy services to patients in their homes, including assessments, goal development, and implementation of therapy plans.
- Work collaboratively with patients, families, and healthcare professionals to ensure seamless care coordination and communication.
- Develop and maintain accurate and comprehensive patient records, including progress notes, treatment plans, and discharge summaries.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
- Stay up-to-date with industry developments and best practices in occupational therapy, and apply this knowledge to improve patient care and outcomes.
- Current Occupational Therapist (OT) licensure in the state of Colorado.
- Current Basic Life Support (BLS) CPR certification.
- One year of OT experience - Home health care preferred.
- Meets the requirements of federal and/or state criminal and abuse background checks where applicable.
- Pass a background check and drug test (we are required to test for THC).
