Job Overview:
The Assistant Director of Care Team Support and Case Management plays a pivotal role in overseeing the operations of care teams, ensuring effective case management practices are upheld across the organization. This position is essential for fostering a supportive environment for both staff and clients.
Key Responsibilities:
- Lead and manage care team initiatives to enhance service delivery.
- Collaborate with various departments to streamline case management processes.

Minimum Qualifications:
