Job Description:

We are seeking a highly motivated and compassionate part-time Medical Respite Care Specialist to join our team at Daily Planet Health Services. This position is a 'float' and supports the overall program schedule, varying each week. As a Medical Respite Care Specialist, you will be responsible for providing support and care to patients who are experiencing homelessness and require medical attention. You will work closely with medical staff to ensure that patients receive the necessary care and support during their stay at the Medical Respite Program. Your role will be critical in helping patients transition from the Medical Respite Program to a stable living situation.

Responsibilities:
  • Provide direct care and support to patients in the Medical Respite Program.
  • Collaborate with medical staff to ensure patients receive appropriate medical care.
  • Assist patients with activities of daily living.
  • Prepare meals and manage kitchen clean-up.
  • Maintain accurate and up-to-date patient records.
  • Assist with discharge planning and coordination of follow-up care.
