Job Summary:
We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a Hospice RN Case Manager, you will be responsible for providing exceptional care to patients and their families, while upholding our high standards of nursing practice.
Responsibilities:
- Maintain a specified case management load, ensuring timely and effective care for patients and their families.
- Conduct comprehensive assessments of patients' physical and psychosocial status, identifying areas for improvement and developing personalized care plans.
- Collaborate with the Interdisciplinary Group (IDG) to develop and implement care plans, ensuring seamless coordination of services and effective communication with patients and their families.
- Provide education and support to patients and their families, empowering them to make informed decisions about their care.
- Participate in on-call duties, ensuring 24/7 coverage and prompt response to patient needs.
- Evaluate and coordinate the need for services, including inpatient, respite, and continuous care, to ensure optimal patient outcomes.
- Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker, ensuring high-quality care and adherence to policies and procedures.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years, or evidence of current clinical knowledge as deemed appropriate by the DHCS.
- One (1) year of hospice care experience, with a strong understanding of hospice principles and practices.
- CPR certification and a valid driver's license, with auto liability insurance.
