Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Box Butte General Hospital. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring the safe and effective use of medications.
Key Responsibilities- Collaborate with physicians, nurses, and other healthcare professionals to provide comprehensive patient care.
- Review and monitor drug therapy to ensure appropriateness, safety, and effectiveness.
- Discuss medication order clarifications with prescribers, document pharmacy records, and inform others of medication order changes.
- Participate in care conferences and swingbed rounds to ensure seamless patient care.
- Assist with medication reconciliations and promote the use of our hospital's formulary.
- Provide patient education on drug therapy and participate in the drug therapy management of medical emergencies.
- Respond to Code Blue emergencies and prepare medications as needed.
- Provide clinically relevant medication-related education to nursing staff and serve on our Medicine/Infection Control/P&T Committee.
- Assist in tracking, analyzing, and presenting hospital antibiogram information and promote antimicrobial stewardship throughout the facility.
- Participate in community outreach projects, including drug takeback and health fairs.
- Track the need for compounded sterile products and mix or check products as needed.
- Double-check drugs before loading them into the Omnicell and assist with billing of medications in our electronic health record software.
- Identify, research, and provide strategies for managing drug shortages.
- Detect, monitor, document, and report adverse drug reactions and medication errors.
- Maintain equipment databases, including IV pumps, medication dispensing cabinets, and electronic health record software.
- Assist the pharmacy manager with responsibilities in accordance with our organization's mission, policy, and applicable laws.
- Actively participate in performance improvement and continuous quality improvement activities.
- Develop policies and processes to ensure compliance with standards of practice from accreditation organizations, including The Joint Commission and CMS.
- Education: Bachelor of Science degree in pharmacy (Doctor of Pharmacy degree preferred).
- Experience: None required, but one year of experience in a hospital setting is preferred.
- License/Certifications: Valid Nebraska Pharmacist license or the ability to become a licensed pharmacist in Nebraska within 90 days of hire.
- Personal Skills: Excellent verbal, written, and interpersonal communication skills, initiative, and analytical ability.
