Box Butte General Hospital is seeking a highly skilled Clinical Pharmacist to join our healthcare team. As a key member of our pharmacy department, you will play a vital role in ensuring the safe and effective use of medications for our patients.
Key Responsibilities- Provide Pharmaceutical Care: Collaborate with physicians, nurses, and other healthcare professionals to provide comprehensive medication management services.
- Review and Monitor Drug Therapy: Evaluate the appropriateness of medication use, identify potential drug interactions, and make recommendations for improvement.
- Communicate with Prescribers: Discuss medication order clarifications, document pharmacy records, and inform others of medication order changes.
- Participate in Care Conferences: Contribute to multidisciplinary care conferences to ensure comprehensive patient care.
- Assist with Medication Reconciliations: Help patients and families understand their medication regimens and identify potential medication-related issues.
- Promote Formulary Use: Convert non-formulary orders to formulary medications when possible and coordinate procurement of non-formulary drugs when necessary.
- Provide Patient Education: Educate patients and families about their medications, including proper use, potential side effects, and medication interactions.
- Participate in Drug Therapy Management: Respond to medical emergencies, prepare medications as needed, and participate in the management of medication-related issues.
- Collaborate with Nursing Staff: Provide clinically relevant medication-related education to nursing staff to ensure safe and effective medication use.
- Participate in Quality Improvement Activities: Contribute to performance improvement and continuous quality improvement activities to enhance patient care and safety.
- Education: Bachelor of Science degree in pharmacy (Doctor of Pharmacy degree preferred)
- License/Certifications: Valid Nebraska Pharmacist license or the ability to become a licensed pharmacist in Nebraska within 90 days of hire
- Personal Skills: Excellent verbal, written, and interpersonal communication skills; ability to work effectively in a team environment; analytical ability to evaluate data and make recommendations; and commitment to supporting the hospital's values of safety, excellence, compassion, integrity, devotion, and teamwork.
