About the Role

In this Geriatric Patient Care Coordinator position, you will be responsible for delivering high-quality care to patients in need. Your primary focus will be on assessment, planning, and implementation of care plans to meet the unique needs of each patient.

You will work closely with a multidisciplinary team to ensure that every patient receives the best possible care. This includes educating patients and support staff on best practices, conducting regular assessments, and making adjustments to care plans as needed.

Key Responsibilities:

  • Patient-Centered Care: Focus on delivering compassionate, high-quality care to geriatric patients, prioritizing their needs and preferences.
  • Care Coordination: Work collaboratively with healthcare teams to develop and implement individualized care plans, ensuring seamless transitions between services.
  • Patient Education: Educate patients and support staff on disease management, medication use, and other relevant topics, promoting informed decision-making.
  • Continuous Quality Improvement: Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of evidence-based practice guidelines.

Requirements:

  • Nursing Background: Proven track record in nursing, with a strong understanding of gerontology and emergency medicine.
  • Collaboration and Communication: Excellent communication and collaboration skills, with the ability to work effectively in a fast-paced environment.
  • Certifications: Possess ACLS and BLS certifications, with a willingness to obtain additional credentials as required by the role.

Salary Range: $90,000 - $115,000 per year, based on experience and qualifications.
