Job Summary

This position is an exciting opportunity to join the Veterans Affairs, Veterans Health Administration team as a Licensed Practical Nurse. As a key member of our healthcare team, you will provide comprehensive nursing care to residents in Long Term Care, dementia unit, respite, hospice, rehabilitation, and transitional care settings.

Responsibilities
  • Provide direct patient care, including medication administration, wound care, and vital sign monitoring.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Document patient information accurately and thoroughly in electronic medical records.
Requirements
  • Graduate of an accredited school of practical nursing.
  • Active, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth of the United States or the District of Columbia.
  • One year of experience as a Licensed Practical Nurse or LVN, with a focus on geriatric care and long-term care.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare teams.
Preferred Qualifications
  • Two years of experience in long-term care or geriatric nursing.
  • Experience with electronic medical records and other healthcare software.
  • Basic Life Support (BLS) certification.
