We are seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will provide high-quality patient care under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians' orders.
Responsibilities:- Provide skilled nursing care to patients in their homes
- Develop and implement individualized care plans
- Conduct thorough assessments and document patient progress
- Collaborate with healthcare teams to ensure seamless care delivery
- Manage tracheostomy and ventilator care for patients
- Graduate of an accredited school of nursing
- Current, unrestricted state license as an LPN
- Current CPR certification
- Demonstrated proficiency in clinical assessments and documentation
- Six months of recent experience as an LPN in a clinical care setting
- Home health experience
