Job Summary

The successful candidate will be responsible for providing high-quality physical therapy services to patients in their homes or community settings.

Key Responsibilities
  • Assess and Evaluate patients to identify their physical therapy needs and develop personalized plans of action.
  • Provide Treatment and interventions to help patients achieve their goals and improve their overall well-being.
  • Document Progress and update care plans as needed to ensure the best possible outcomes.
  • Collaborate with Physicians and other healthcare professionals to ensure comprehensive care.
  • Monitor Quality and appropriateness of services provided to ensure the highest level of care.
Requirements
  • Physical Therapy Licensure in New Jersey is required.
  • 2-3 years of prior clinical experience and community-based experience are preferred.
  • Strong Computer Skills and excellent communication and interpersonal skills are essential.
  • Valid NJ Driver's License is required.
