BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Palisades Park, New Jersey.

We are looking for an experienced Physical Therapist to provide high-quality care to our clients in their homes. As a member of our multidisciplinary team, you will work closely with nurses and other healthcare professionals to develop and implement individualized care plans.

Responsibilities include:

  • Conducting home visits to assess clients' physical needs and develop personalized care plans.
  • Implementing evidence-based interventions to promote wound healing, improve mobility, and enhance overall physical function.
  • Collaborating with the healthcare team to revise and update care plans as needed.
  • Educating clients and their families on rehabilitative care and activities to promote independent living.
  • Accurately documenting client interactions, interventions, and outcomes using our state-of-the-art electronic medical records system.

Qualifications:

  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
