Job Title: Health Home Adult Care Manager Supervisor

At Jemcare, we are seeking a highly skilled and experienced Health Home Adult Care Manager Supervisor to join our team. This is a full-time position that requires a proactive leader who can balance clinical expertise with strong management skills.

Key Responsibilities:
  • Team Leadership and Supervision: Lead a team of six Team Leads who supervise eighty adult Care Managers, offering mentorship, guidance, and performance evaluations.
  • Program Oversight: Monitor care management activities for compliance with state and organizational guidelines, including Health Home and Medicaid regulations.
  • Care Coordination and Quality Assurance: Oversee the development and evaluation of person-centered care plans for adult clients.
  • Training and Development: Provide ongoing professional development opportunities focused on motivational interviewing, trauma-informed care, and chronic disease management.
  • Client and Community Engagement: Act as a liaison between Care Managers, clients, and external partners to promote integrated care coordination.
  • Data Management and Reporting: Oversee quality metrics and reporting, ensuring accurate tracking of client outcomes and service utilization.

Requirements:

  • Bachelor's degree in Social Work, Human Services, Public Health or a related field required.
  • Minimum of 3 years of experience in care management, care coordination, or case management in a healthcare or social services setting.
  • At least 1 year of supervisory or leadership experience preferred.
