Key Responsibilities
  • Provide exceptional customer service and support to clients, responding to their inquiries and concerns in a respectful and empathetic manner.
  • Assess client needs and provide guidance on available resources and services, including Medicaid waiver programs.
  • Develop and maintain relationships with community partners and stakeholders to ensure seamless referrals and support.
  • Manage and maintain accurate records and databases, ensuring timely and efficient data entry and reporting.

Requirements
