POSITION SUMMARY
The Oncology Care Coordinator will be dedicated to supporting patients diagnosed with breast cancer, ensuring a comprehensive approach to their care.

KEY RESPONSIBILITIES:
  • Facilitate the management of patient care across various stages in partnership with healthcare professionals and the multidisciplinary team, aiming for a streamlined experience for oncology patients.
  • Act as an advocate, educator, and clinical resource, possessing in-depth knowledge in medical, surgical, and radiation oncology as well as care management.
  • Serve as a key point of contact within the facility and the community, providing information about services available for this specialized patient group.
  • Oversee all elements of the proposed treatment plan and regimen.

PHYSICAL DEMANDS:
Essential:
Requires finger dexterity, visual acuity, auditory perception, verbal communication, and the ability to stand, sit, and walk.

Frequent:

Involves pushing/pulling weights up to 50 pounds, carrying weights between 10 to 15 pounds, and repetitive arm/hand movements.


Occasional:

May include bending, climbing stairs, navigating uneven surfaces, squatting, twisting, and lifting weights of up to 75 pounds with assistance, as well as reaching at various levels.


  • Proficient in operating physiological monitoring devices, computers, printers, copy machines, and typewriters.

WORKING CONDITIONS:
  • Typically not exposed to significant adverse environmental factors.
  • The work environment is dynamic due to multiple demands.
  • Flexibility in working hours may be necessary to accommodate departmental requirements.
  • Occasional travel may be required.

MINIMUM QUALIFICATIONS:
EDUCATION AND LICENSURE:
  • Valid Hawaii State License as a Registered Nurse.
  • Bachelor’s degree in Nursing.
  • Preferred certification in Oncology Nursing.
  • Current certification in BLS/HCP.

EXPERIENCE:
  • Minimum of two years of nursing experience in outpatient and/or acute care settings.
  • Proven skills in patient education.
  • Demonstrated experience in performance improvement initiatives.
  • Familiarity with electronic medical records systems.
