We are seeking a highly skilled Clinical Oncology Pharmacist to join our team at Hawaii Pacific Health. As a key member of our pharmacy department, you will play a critical role in ensuring the safe and effective medication treatment of our patients.
Key Responsibilities- Provide comprehensive medication management for hospitalized oncology patients, focusing on antineoplastic medications and complex disease states.
- Collaborate with physicians, mid-level providers, nurses, and other healthcare staff to ensure continuity of care.
- Review laboratory results and medication orders, monitor protocol adherence, and ensure quality control and integrity.
- Participate in multidisciplinary team rounds, oncology pharmacy and therapeutics initiatives, system protocol development, and other committee activities.
- Provide drug education and training to healthcare staff.
- Must meet education requirements for Hawaii state licensing.
- Current Hawaii Pharmacist license.
- Two (2) years inpatient pharmacist or related experience or completion of pharmacy residency training.
- PharmD degree and pharmacy residency training.
- Clinical pharmacist experience.
