We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to patients in their homes. As a LPN Homecare Specialist, you will have the opportunity to work with a variety of patients, including those with gastrostomy tubes, tracheostomy tubes, and mechanical ventilators.
Responsibilities- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Perform focused interviews to identify specific patient needs.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
- Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
- Current Ohio Licensed Practical Nurse (LPN) License
- Active CPR/BLS Certification (American Heart Association or American Red Cross)
