Your Role:
As a Clinical Liaison Registered Nurse:
You will report directly to the AVP of Business Development.
Our Clinical Liaison Registered Nurses are integral to our exceptional team, responsible for:
Ensuring the intake and referral processes are executed efficiently and promptly to meet the needs of patients, families, and referral sources
Completing the admissions process for referred patients in urgent situations, as staffing allows, and for inpatient hospice admissions
Providing accurate and timely information regarding hospice services to all inquiries
Responding promptly to calls from referral sources and initiating necessary paperwork to commence the intake/referral process
Acting as a patient advocate in assessing and developing care plans and obtaining informed consent for hospice services
Engaging with various account types, including Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), hospitals, and physicians
Delivering in-services and educational sessions on the advantages of hospice care
Requirements for Hospice Clinical Liaison Registered Nurse:
Compassionate Heart:
Demonstrates kindness, empathy, and compassion towards clients, their patients, and families.
Must hold a valid RN license in the state(s) of employment
Preferred minimum of 2 years of healthcare experience in settings such as hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planning, hospice case management, or RN education
Well-known and respected within the community
