Main Street Health is a leading provider of value-based care in rural America. We're seeking a skilled Health Navigator to join our team and make a meaningful impact in the lives of our patients.
The Health Navigator will play a critical role in helping our patients navigate the complexities of the healthcare system. This will involve developing trusted relationships with patients and providers, and assisting with various patient-facing, administrative, and logistical tasks.
As a Health Navigator, you will work closely with our patients to understand their needs and provide personalized support. You will also work with our providers to ensure that patients receive the care they need, when they need it.
The ideal candidate will be a strong communicator with excellent interpersonal skills. You will be comfortable working in a fast-paced environment and be able to prioritize multiple tasks and responsibilities.

Requirements:
- Bachelor's degree in a related field
