Job Title: Registered Nurse-Home Health

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health RN for a travel assignment in a dynamic and supportive environment. As a key member of our healthcare team, you will provide high-quality patient care and education to patients in their homes, promoting independence and well-being.

Responsibilities:
Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
Develop and implement individualized care plans to meet the unique needs of each patient.
Collaborate with healthcare team members to ensure seamless care coordination and communication.
Educate patients and their families on disease management, medication adherence, and healthy lifestyle habits.

Requirements:
Current RN licensure in the state of Minnesota.
Minimum 1 year of experience as a Home Health RN.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
