About the Role

We are seeking a dedicated and compassionate After Hours Nurse to join our team at Hospice of the Western Reserve. As an After Hours Nurse, you will play a vital role in providing high-quality care to patients and their families during critical times.

Responsibilities
  • Assess the physical, psychosocial, and spiritual needs of patients and families.
  • Provide intermittent skilled nursing services to patients under the direction of their physician.
  • Monitor and evaluate pain and symptoms, and provide information and assistance to patients and families to manage those problems.
  • Serve as a liaison to patients' physicians when needed, and provide information to physicians based on knowledge related to patients' needs.
  • Provide emotional support to patients and families.
  • Refer patients and/or family members to other services when appropriate.
  • Provide instruction (healthcare teaching) to patients and families as related to care in a residence.
  • Make visits to patients' residences at the time of death.
  • Maintain, promptly complete, and return all documentation as required.
  • Communicate and act as a liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
  • Relay pertinent information to appropriate team members.
  • Participate in meetings as initiated by team leaders.
  • Participate in mandatory or elective HWR University events, trainings, or in-services as necessary.
  • RNs shall participate in triage coverage as needed.
  • Perform other duties as assigned.
Requirements
  • High School Diploma or equivalent required.
  • Graduate of an accredited school of nursing, with a Bachelor's degree preferred.
  • Licensed RN/LPN in the state of Ohio.
  • Experience preferred in pain and symptom management, oncology, or mental health.
  • Ability to communicate effectively, orally and in writing.
  • Ability to work independently and collaboratively with a team.
  • Must have demonstrated knowledge and skills to provide appropriate care to all age groups.
  • Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere.
  • Ability to access patients and families in any residential and/or public setting in any weather condition.
  • Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals, and virtual meeting technology.
  • Must have and maintain a valid driver's license, good driving record, and auto accident insurance at a level acceptable to HWR and/or HWR's insurer.
Physical Requirements
  • Ability to stand and walk for extensive periods of time.
  • Manual and finger dexterity and eye-hand coordination to provide nursing care and operate equipment.
  • This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently utilizing appropriate safe transfer techniques and body mechanics when working with patients.
  • Ability to stoop, squat, or bend frequently.
  • Corrected vision and hearing within normal range to observe and communicate with patients and professional staff.
