About This Opportunity

We are seeking a skilled and compassionate After Hours Nurse to join our team at Hospice of the Western Reserve. As an After Hours Nurse, you will play a critical role in providing high-quality care to our patients and their families during times of need.

Key Responsibilities
  • Assess the physical, psychosocial, and spiritual needs of patients and families, and develop individualized care plans to address these needs.
  • Provide intermittent skilled nursing services to patients under the direction of their physician.
  • Monitor and evaluate pain and symptoms, and provide education and support to patients and families to manage these issues.
  • Serve as a liaison between patients, families, and healthcare professionals, and communicate effectively to ensure seamless care coordination.
  • Provide emotional support and counseling to patients and families, and refer them to additional resources as needed.
  • Document all patient interactions and care provided, and maintain accurate and up-to-date records.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Graduate of an accredited school of nursing, with a Bachelor's degree preferred.
  • Licensed RN or LPN in the state of Ohio.
  • Experience in pain and symptom management, oncology, or mental health preferred.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality care.
  • Valid driver's license and good driving record, with the ability to travel to patient homes and other locations as needed.
