At Capitol Home Health, we are seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes. As a Physical Therapist, you will be responsible for evaluating and assessing patient needs, developing treatment plans, and providing therapy services in a home care environment.
Key Responsibilities:
- Evaluate and assess patient needs to develop effective treatment plans
- Provide physical therapy services in a home care environment
- Work cooperatively with physicians and case managers to ensure patient care
- Use creativity and latitude to provide individualized care
Requirements:
- Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy
- Current and unencumbered licensed to practice as a Physical Therapist in the state of employment
- Two years of experience as a Physical Therapist, with community/home health experience preferred
- Demonstrates good verbal and written communication and organization skills
- Possess and maintain valid CPR certification
- Access to reliable transportation
