Job Summary:

VibrantCare Rehabilitation seeks a skilled Physical Therapist to implement high-quality physical therapy services for patients, caregivers, and facilities in accordance with physical therapy principles and practices. The ideal candidate will develop treatment plans, supervise assistants and aides, contribute to case management, and provide quality patient care while maintaining positive interactions with facilities and clients.

The successful candidate will understand and adhere to state Practice Acts, review and adhere to the VibrantCare Clinical Operations Policies & Procedures Manual, and complete required trainings and certifications.

Key responsibilities include evaluating, treating, and directing skilled treatment for patients, maintaining accurate documentation, and communicating effectively with co-workers, physicians, patients, and third-party payors.

The Physical Therapist will also participate in facility meetings, complete continuing education coursework, and maintain current CPR certification.

Essential Functions:

  • Understand and adhere to state Practice Acts and laws governing physical therapy practice and supervision.
  • Review and adhere to the VibrantCare Clinical Operations Policies & Procedures Manual.
  • Complete required trainings and certifications, including Medicare compliance and OSHA training.
  • Evaluate, treat, and direct skilled treatment for patients.
  • Maintain accurate documentation and communication with co-workers, physicians, patients, and third-party payors.
  • Participate in facility meetings and complete continuing education coursework.
  • Maintain current CPR certification.

  • Ability to work effectively as a team member.
  • Leadership skills to direct and supervise assistants and aides.
  • Strong organizational, time management, and planning skills.
  • Effective clinical problem-solving skills.
  • Strong communication skills for accurate, concise, and organized oral and written presentations.
  • Computer skills for effective clinical documentation and management reporting.

Physical Requirements:

  • Ability to lift patients using appropriate biomechanical techniques.
  • Ability to walk, bend, stand, and reach constantly during a work day/shift.
  • Visual acuity sufficient to evaluate, diagnose, and monitor patient needs.
  • Ability to speak and hear sufficiently to understand and give directions.
  • Ability to push wheeled equipment through the facility and in the community.

Required Credentials:

  • Graduation from a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).
