At Center for Elders' Independence, we are seeking a highly skilled and compassionate Physical Therapist to join our interdisciplinary team. As a Physical Therapist, you will play a vital role in providing restorative and rehabilitative services to our elderly participants, helping them to maintain their independence and quality of life.
Key Responsibilities:- Collaborate with the primary care provider team and interdisciplinary team to evaluate participants and develop individualized plans of care.
- Provide physical therapy services, including exercise, soft tissue and joint mobilization, functional training, and neuromuscular re-education.
- Work with vendors to obtain rehabilitative equipment and supplies for participants.
- Conduct initial assessments and reassessments as needed, and record and report on participants' progress.
- Supervise and evaluate therapy program aides, and provide in-service training to other members of the health team.
- Attend interdisciplinary team conferences and participate in the development of the plan of care.
- Evaluate participants' home environments and make recommendations as needed.
- Participate in continuing education and maintain required certifications.
- Possession of a minimum of a Bachelor's degree in Physical Therapy from a program approved by the Commission on Accreditation in Physical Therapy Education.
- Current licensure as a Physical Therapist by the Physical Therapy Board of California.
- Preferred, one year of experience working as a Licensed Physical Therapist with the frail elderly population.
- Current CPR and First Aid certification on file.
- A valid California driver's license, current vehicle insurance, and access to a reliable vehicle.
We offer a competitive hourly rate of $47.76 to $71.62 per hour, based on market rate, experience, skills, abilities, and work history. This position is eligible for a 7,500 sign-on bonus.
